Documents- Post Adoption Prospective Clients

 
In addition to the completed forms, we also need a copy of the adoption decree, which verifies consummation of the adoption, along with proof of Texas Department of Family and Protective Services (TDFPS) conservatorship.  Proof of conservatorship should be stated in the adoption decree; however, if it is not stated that TDFPS was the conservator, please send a copy of your Adoptive Placement Agreement or proof of Adoption Assistance.

These documents must be returned within 30 days and forms must be received in order to be eligible to participate in the Post Adoption program, including the use of respite reimbursement and attendance at DePelchin sponsored events.

Once enrolled, requirements for continuation in Post Adoption services as directed by the licensing agencies, Council on Accreditation (COA) and TDFPS:

  • Schedule initial intake assessment/interview within five (5) working days after receipt of intake packet.
  • Parent signs and returns Plan of Service every six (6) months including face-to-face visits (at least twice per year) within a month of plan of service due date.
  • Family maintains contact with assigned clinical case manager on a monthly basis (via telephone, letter, email, etc.)

*Also include a copy of your child’s current psychological evaluation, if available, for your Clinical Case Manager’s review.*
